In addition, you can make a request to be provided with copies of your health record by submitting a Subject Access Request (SAR) form; A copy of the SAR form is available from reception. You will need to return the completed paper copy of the SAR to the practice. Patients do not have to pay a fee for copies of their records. Once the SAR form is submitted, Downton Road Surgery will aim to process this request within 21days, however, this may not always be possible. The maximum time permitted to process SARs is 28 days.
